Instructions for Use of the FILE FOR LIFE

• Fill out the medical card and be sure information is accurate and legible.

• Use a pencil anywhere information may frequently change - like medications. This will allow for any updates you may need to make in the future.

• When completed, place the card in the pouch and affix to the outside of your refrigerator.

• Direct First Responders to the File of Life located on your refrigerator.

• Take the File of Life with you when you visit your doctor.
